Assessments can now be completed via a Healthnote - a link that is sent prior to your visit!

Our pediatricians and nurse practitioners would like to share with our patients a service we use to better serve our patients for office visits. We take the health of your child very seriously and want to use all the available tools that will assist in providing the best care possible for your child. Therefore, we would like you to register for this set of web-based questionnaire tools.

Healthnote will be used for patient registration, insurance verification, co-pay collection, well visit and office visit developmental, behavioral and medical assessments. Healthnote will override the Athena online check in system. Healthnote can be accessed from the comfort of your mobile device (no password needed) prior to your appointment. We are asking all of our patients to use Healthnote to streamline the check in process, you will receive a link to Healthnote within two weeks prior to your visit or same day for same day sick appointments.
